Job Overview:

We are seeking a highly skilled and compassionate Nurse Practitioner to join our dynamic team as a Long-Term Care Nurse Practitioner. Responsibilities:

* Conduct comprehensive assessments of patients in long-term care facilities, including medical history, physical examinations, and medication reviews.

* Develop and implement individualized care plans to address the unique needs of long-term care residents.

* Collaborate with interdisciplinary healthcare teams to coordinate and optimize patient care.

* Provide education and support to patients, families, and caregivers on disease management, preventive care, and overall health and wellness.

Qualifications:
