Position: Nurse Practitioner - Permanent Full-Time

Permanent | 40 Hours/Week | Starting at $190,000 | HOOPP Pension | Established Patient Panel

ByWard Family Health Team (BWFHT) is seeking an experienced Nurse Practitioner (NP) to join our interdisciplinary primary care team in a permanent, full-time role.

This is an opportunity to take ownership of an established patient panel of approximately 1,700 patients, build meaningful, long-term relationships with your patients, and practise comprehensive primary care with a high degree of clinical autonomy.

You won't be doing it alone. You'll be supported by physicians, nurses and a diverse team of allied health professionals, giving you access to additional expertise and services to support the medical, mental health and social needs of your patients.

The position is 40 hours per week with a starting annual salary of $190,000, comprehensive benefits and enrolment in the Healthcare of Ontario Pension Plan (HOOPP).

THE ROLE

As a Nurse Practitioner with BWFHT, you will serve as the primary care provider for an established panel of approximately 1,700 patients, providing comprehensive and longitudinal primary care.

You will independently assess, diagnose, treat and manage a broad range of acute and chronic health conditions while working collaboratively with physicians and our interdisciplinary team.

Key Responsibilities
  • Provide comprehensive, longitudinal primary care to your assigned patient panel.
  • Assess, diagnose and manage acute and chronic health conditions within the NP scope of practice.
  • Order and interpret investigations, prescribe medications and develop appropriate treatment plans.
  • Provide preventative care, screening, health promotion and patient education.
  • Coordinate referrals and follow-up with specialists and community providers.
  • Collaborate with physicians, nurses and allied health professionals to provide coordinated, team-based care.
  • Maintain timely and accurate clinical documentation and contribute to quality improvement initiatives.

WHAT YOU BRING
  • Current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class (Nurse Practitioner).
  • Graduate of a recognized Nurse Practitioner program.
  • Current professional liability protection.
  • Minimum of two years of Nurse Practitioner experience, preferably in primary care.
  • Strong assessment, diagnostic and clinical decision-making skills.
  • Experience managing acute and chronic health conditions.
  • Ability to independently manage a primary care patient panel.
  • Strong communication, organizational and time-management skills.
  • A collaborative approach to interdisciplinary care.
  • Experience working with an electronic medical record (EMR).
  • Bilingualism in French and English is an asset.
