Description

Job Title: Nurse Practitioner PACE

Job Summary

Under the general supervision of the medical director of FLORIDA PACE CENTERS, provides direct primary medical care treatment, and counsel to all participants enrolled into the program.

Essential Job Functions
• Performs comprehensive history and physical on new referrals. Performs interval history and physical on established participants done quarterly or as indicated.
• Manages patients with acute, episodic, and chronic illnesses.
• Assumes ongoing responsibility for each patient under her/his care.
• Refers participants to medical director or medical specialist as indicated
• Admits participants to the hospital, provides primary care in conjunction with Medical Director, provides updates to multidisciplinary team on a regular basis, judiciously utilizes special consultants and coordinates discharge planning with team.
• Manages care of participants in the nursing home through regular visits, telephone contact with nursing home staff and admits nursing home participants to hospital when necessary.
• Actively participates in family conferences. Attends in interdisciplinary team meetings and contributes to the interdisciplinary team.
• Participate in committees and work groups as assigned.
• Documents treatment in patient charts according to established department guideline.

Job Requirements

Education:
• Graduate from an accredited school of nursing
• Advanced training and certification as an Advanced Registered Nurse Practitioner (ARNP) or advanced training and licensure as a Physician Assistant (PA).

Experience:
• 2-3 years' experience preferred; minimum of 1 year specifically working with frail elderly population.

Licenses/Certifications:
• Current ARNP/PA license by the Florida State Board of Nursing
• BLS certification required
• ACLS certification preferred
• Drug Enforcement Administration (DEA) License required

Abilities required:
• Knowledge of nursing care practices for rehabilitation, geriatric and dementia patient populations preferred.
• Demonstrates a caring attitude toward residents, patients, family members, employees, and other facility guests

Abilities Required

Environment Work Conditions:
• Normal working condition with adequate lighting and ventilation
• Exposure to toxic or caustic chemicals; bio-hazardous waste; flammable, explosive gas
• Ability to assist transferring patients and ambulation which may involve lifting, pushing, and pulling of patients from 50-150 pounds.

Infectious Material Exposure:
• Routine exposure to blood/body fluids/tissues
