Job DescriptionAtlantic Health is Seeking a Diabetes Nurse Practitioner at Summit, New JerseyThe NP will support both outpatient services at 11 Upper Overlook Road, MAC II, Suite 195, Summit, NJ, as well as inpatient services at Atlantic Health Overlook Medical Center in Summit, NJ.
As a Diabetes NP, you will have a crucial role in delivering specialized diabetes care to adult patients aged 18 and above. Your responsibilities will include providing comprehensive diabetes care to patients, ensuring high-quality, evidence-based practice. Collaborating with a multidisciplinary team, you will develop and implement individualized treatment plans, educate, and support patients in self-management of their conditions, and contribute to the growth and advancement of the Endocrine program through active participation in quality improvement initiatives. Additionally, you will engage in continuous learning and professional development to stay updated with the latest advancements in the field.

- Master's degree required
- Certified Diabetes Counselor Education Specialist (CDCES), or ability to obtain a certification within 1 year of hire
- ACLS and BLS certification required
- Preferred minimum 1-2 years' experience in acute care and/or physician practice setting with diabetes patients
- Must have excellent communication and computer skills
- Strong work ethic and a team player who can work independently
