Job Function: - Schedule: Monday to Friday.
- Deliver comprehensive outpatient medical care for adults, including wellness visits, health assessments, preventive screenings, and management of ongoing medical conditions.
- Evaluate, diagnose, and treat a wide range of acute illnesses and chronic health concerns.
- Work closely with physicians, advanced practice providers, nurses, behavioral health professionals, and other care team members to ensure coordinated treatment plans.
- Educate patients on healthy lifestyle choices, preventive healthcare strategies, medication adherence, and self-management of chronic diseases.
- Accurately document patient encounters, treatment plans, and clinical outcomes within the electronic medical record system.
- Support organizational quality initiatives and actively participate in clinical meetings and performance improvement activities.
- Perform other responsibilities and special projects as assigned by leadership.
Job Requirements:Job Requirements: - Medical degree (MD, DO, NP, or PA) from an accredited institution
- Board certification or eligibility in Internal Medicine or Adult Primary Care
- Current New York State license or eligibility for licensure
- Experience in community health or outpatient primary care preferred
- Excellent communication and interpersonal skills
- Bilingual Creole/English strongly preferred
