The Nurse Practitioner applies advanced education and clinical competencies to achieve patient outcomes. The Nurse Practitioner works on problems of diverse scope and complexity ranging from moderate to substantial.

The Integrated Home Care Program is a creative initiative dedicated to transforming healthcare delivery for high-risk patients in the comfort of their own homes. We aim to reduce avoidable hospitalizations and readmissions by providing comprehensive, patient-centered care to individuals with medical complexity and functional limitations. Our team delivers proactive, coordinated care directly where our patients need it most.

Main Responsibilities:

  • You will conduct comprehensive in-home evaluations and examinations
  • You will use both in-person and telemedicine capabilities to deliver care
  • You will develop and implement personalized treatment plans
  • You will manage acute clinical issues in the home
  • You will maintain accurate documentation
  • You will participate in medical team meetings

You will report to our Chief Medical Officer for San Antonio, Texas. As part of this role, you will work onsite visiting patient's homes in San Antonio, Texas.

If selected for this role, we will require you to undergo a TB screening test.

Required qualifications:

  • Nurse Practitioner or Physician Assistant with 1 + years of experience in Home Health, or a related field.

Preferred qualifications:

  • Bilingual in Spanish and English

Additional Information:

  • Language Proficiency Testing: any associate who speaks with a member in a language other than English must take a language proficiency assessment, provided by an outside vendor, to ensure competency. Applicants will be required to take the Interagency Language Rating (ILR) test as provided by the Federal Government.

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.$123,800 - $170,300 per year


 

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
