Job Summary:
The Nurse Practitioner shall be licensed in the State of Mississippi and must practice according to a Board approved protocol which has mutual agreement of and by a Mississippi licensed physician and the approval by the Board of Trustees. He/She is directly accountable to the Medical Director of the Skilled Nursing Unit and the Executive Director of Post-Acute Care. Confers with the Medical Director on assigned patients. The nursing process will be utilized (assessment, planning, intervention, evaluation and teaching) in providing patient care. Keeps physician informed of needs and problems related to patient care. Implements and establishes policies and procedures for quality measures, outcomes, safety, environmental and infection control. Utilizes supplies and equipment in a cost effective manner. The Nurse Practitioner will demonstrate knowledge and skills necessary to provide care appropriate to the age of the patient served; also demonstrate knowledge of the principles of growth and development and process data reflective of the patient's status. The Nurse Practitioner performs beside care with patient and family evaluations, assists with diagnosis, treatment and management of care as appropriate. Complies with applicable laws and regulations to the scope of advanced practice nursing and Skilled Nursing Unit regulatory guidelines. Performs other job duties as assigned.
Performance Expectations:
- Collaboration with the Medical Director and other members of the Interdisciplinary Team to assess and evaluate patients in the Skilled Nursing Unit.
- Primary duties are assisting in daily rounds and providing direct patient care for Skilled Nursing Unit patients to include comprehensive assessments, physical exams, subsequent visits, formulation of treatment plan and discharge visits.
- Keep Medical Director informed as appropriate of needs and problems related to patient care.
- Discharge patients in collaboration with Interdisciplinary team and Medical Director as appropriate.
- Order and supervise diagnostic tests after collaboration with physician.
- Provide appropriate education to both patient and family members.
- Order and review lab, radiology and other diagnostic tests in collaboration with Medical Director.
- Document assessments/visits in electronic medical record along with treatment plan as appropriate.
- Collaboration with physician on admission orders
- Make referrals to appropriate ancillary services as applicable
- Complies with established standards, policies, procedures
- Maintains a clean, orderly, safe environment for patients and personnel.
- Maintain patient rights and confidentiality of patient information
- Demonstrate knowledge and skills to appropriately communicate and interact with the patients, families, and visitors of all age groups while being sensitive to their cultural and religious beliefs.
Qualifications:
Education/Skills
Graduate of a recognized advanced practice RN program required.
Work Experience:
Previous work experience of at least 2 years as a Nurse Practitioner in long term care, skilled nursing units preferred
Certification/Licensure-DUE UPON HIRE
Licensed as an APRN by Mississippi State Board of Nursing required.
Certification as a Family Nurse Practitioner by the American Nurse Credentialing Center (ANCC) required.
