Schedule: Full-time

Base salary: $117,451.00 - $131,714.00 per year

CME Allowance: $3,000

What You Will Do

This is an exceptional opportunity for a motivated NP/PA to join an employed position within a busy, well-respected practice, supported by a strong referral base and the full resources of a robust health system network.

The Advanced Heart Failure APP will work as part of a multidisciplinary team providing comprehensive care to patients with heart failure, including those receiving guideline-directed medical therapy (GDMT) and advanced heart failure therapies. This role is primarily involves close collaboration with cardiologists, nurses, pharmacists, and care management teams to optimize patient outcomes and reduce hospitalizations.

  • Provide comprehensive evaluation and management of heart failure patients in the outpatient/Inpatient setting.
  • Optimize and titrate guideline-directed medical therapy in accordance with current heart failure guidelines.
  • Coordinate transitions of care and timely follow-up following hospitalizations.
  • Collaborate with physicians and multidisciplinary team members to develop individualized treatment plans.
  • Educate patients and caregivers on disease management, medications, diet, and lifestyle modifications.
  • Monitor patient progress, review diagnostic testing, and adjust treatment plans as appropriate.
  • Participate in quality improvement initiatives and evidence-based practice efforts within the Heart Failure Program.
  • Maintain accurate and timely documentation and support program quality metrics.
  • Provide inpatient heart failure rounding coverage every fourth weekend to support continuity of care.
  • Prior Cardiology and/or Heart Failure experience preferred
  • Board Certified Nurse Practitioner or Physician Assistant
  • Eligible for state medical license and DEA/Controlled Substance registration
  • BLS and ACLS certification required

Schedule

  • Nine half-day outpatient clinic sessions per week.
  • Inpatient heart failure rounding every fourth weekend.

What You Will Need

Licensure / Certification / Registration:
  • BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
  • Nurse Practitioner credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date required.
  • Drug Enforcement Admin credentialed from the Drug Enforcement Administration preferred.

Education:
  • Master's degree of Nursing required.
