Job Description

The RN Stroke Navigator will be the first contact the patient before hospital discharge and then follow the patient after discharge providing coordination between the patient, PCP, neurologist and other specialists. This position will perform in hospital early post discharge screening for medication compliance, healthy life style, depression, medical complications, and caregiver burden while all the while being the patients advocate and anticipating their needs.

Required Qualifications:
  • Bachelor's Degree in Nursing; or obtained within 3 years.
  • Upon hire: Oregon Registered Nurse License
  • Upon hire: National Provider BLS - American Heart Association
  • 5 years of related neurological clinical experience.

Preferred qualifications:
  • Master's Degree in Nursing.
  • Upon hire: NIHSS Certification
  • Upon hire: Stroke Certified Registered Nurse
  • Upon hire: American Association of Neuroscience Nursing membership and certification
  • 1 year of experience in an Ambulatory setting.
  • Experience working with patients who have neurological conditions, and their families.
  • Experience working with a Joint Commission Certified Stroke Center.

Posted are the minimum and the maximum wage rates on the wage range for this position. The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
