Job Overview

In partnership with the Pharmacy Leader and other stakeholders, provide workflow management of the day-to-day operations in Rheumatology, driving patient care excellence while ensuring achievement of organizational goals. Is a key pharmacy liaison between service lines, programs, domains, and clinical areas as assigned. Foster an environment that includes consistent excellent patient and customer experience, outcomes that meet or exceed quality standards and financial targets, a culture of employee engagement and ongoing organizational and regulatory compliance.

Responsibilities
  • Leads the day-to-day operations of assigned MTM department/area to ensure achievement of organizational vision, mission, and goals.
  • Provide clinical, formulary and pharmacotherapy expertise and act as a resource to students, staff, and interdisciplinary members of the care team. This may include coordinating student experiential rotation and acting as preceptor.
  • Provide MTM services to all patients in assigned area using Standard Operating Procedures (SOP) or practice model. This may include providing clinical services for patients, monitoring, adjusting patient therapies and documenting changes according to department policies and pharmacy regulations.
  • Act as resource for pharmaceutical care to patients, partnering with interdisciplinary care team members to ensure the appropriateness of medication order and therapy and the effectiveness, safety, and convenience of the medication therapy.
  • Develop, implement, and communicate key metrics and improvement activities to monitor the performance of pharmacy and/or clinical operations.
  • Responsible for timely and accurate analysis and decision making to support pharmacy business performance, operational and financial objectives.
  • Drive a culture of continuous improvement using lean management tools to support, challenge and evolve processes to achieve system pharmacy goals.


Required Qualifications
  • Pharmacist Degree
  • 1 year pharmacist experience in complex pharmacy operation, clinical service area or completion of an ASHP-accredited PGY1 residency program
  • Certification in Pharmaceutical Care as approved by Fairview Pharmacy Services Management.
  • Basic Life Support (American Heart Assoc or Red Cross) within 90 Days
  • Pharmacist MN Board of Pharm

Preferred Qualifications
  • Doctorate of Pharmacy
  • Completion of an ASHP-accredited PGY1 residency program
  • 3 years experience in a specialty setting
  • Minnesota Board of Pharmacy Preceptor License
  • Ambulatory Pharmacy Board Certification (BCACP), or other accredited board certification
