Minimum Data Set RN at Providence St. Joseph Medical Center in Burbank, CA. This position is Full Time and will work 8-hour Day shifts.

The Rehabilitation PPS (Perspective Payment System) Coordinator is responsible for the overall comprehensive data collection of the patient as the foundation for planning and delivering care to the patient. The PPS Coordinator oversees the accuracy and the completion of the Inpatient Rehabilitation Facility Patient Assessment Instrument (IRF-PAI) and inputs data for electronic transmissions of this information to the government. Monitors and coordinates the interdisciplinary plan of care, intake, discharge planning, and continuing post-acute care needs of the patients assigned.

Required qualifications:
  • Degree for completion of LVN, RN, PT, OT, or SLP. Or Bachelor's degree.
  • Upon hire: California Registered Nurse License.
  • 5 years In hospital environment.
  • 3 years Rehab related experience in hospital or community setting.

Preferred qualifications:
  • Coursework/Training: Advanced training/certification in computer skills and medical coding.
  • 1 year Case management in IRF or PPS Coordinator.

These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
