Summary:
The Community Health Outreach & Events Manager is responsible for overseeing Equal Hope's Community Health Workers (CHWs), coordinating community outreach and education efforts, leading outreach-related communications, and planning and executing community events that advance Equal Hope's mission to eliminate health inequities in Chicago and surrounding counties.
This role ensures that CHW activities, outreach messaging, and community events are aligned, data-informed, and culturally responsive to the communities Equal Hope serves.

Required Job Qualifications:
• 4-6 years of progressively responsible experience in community health, outreach, CHW programs, or related public health/community-based work, including at least 1-2 years in a supervisory or lead role.
• Demonstrated experience planning and implementing community outreach and events in diverse, under-resourced communities.
• Strong written and verbal communication skills, with experience creating accessible, community-facing materials and/or health education content.
• Ability to work effectively with diverse stakeholders, including community members, CHWs, healthcare providers, and partner organizations.
• Proficiency with standard office software and basic data tracking (e.g., Microsoft Office/Google Suite, databases/CRMs, spreadsheets).
Preferred Job Qualifications:
• Bachelor's degree in public health, social work, community health or a related field or an equivalent combination of education, training, and experience[JC2.1].
• Lived experience in communities most impacted by health inequities in the Chicago region.
• Bilingual (e.g., English/Spanish) strongly preferred.
• Previous experience as a Community Health Worker or supervising CHWs.
Physical Demands:
• Requires frequent travel throughout Chicago and surrounding counties (including Cook, Lake, and Will counties) to attend meetings, outreach events, and partner sites; some evenings and weekends are required for community events. A valid driver's license and reliable personal vehicle are required, with mileage reimbursed in accordance with Equal Hope policy.

Responsibilities:
• Provide day-to-day supervision, coaching, and support for the CHW team, including regular check-ins, case discussions, and performance feedback.[JC1.1][JC1.2]
• Develop clear work plans, outreach targets, and workflows for CHWs in collaboration with leadership and program directors.
• Monitor CHW documentation, data collection, and reporting to ensure accuracy, timeliness, and alignment with grant and contract requirements.
• Identify training needs and coordinate onboarding, in-service trainings, and professional development opportunities for CHWs.
• Foster a supportive, trauma-informed, and culturally responsive team culture that reflects the communities served, including Black and Latine communities across Cook, Lake, and Will counties.
• Lead development and implementation of an annual outreach plan, including goals for events, community presentations, and partnerships that support access to breast and cervical cancer screening and other priority services.
• Maintain and manage a master calendar of community outreach activities, health fairs, presentations, and tabling events across Equal Hope's service area.
• Build and strengthen relationships with community partners (e.g., FQHCs, hospitals, CBOs, faith communities, schools, local governments) to expand Equal Hope's reach and impact.
• Represent Equal Hope at community meetings, coalitions, and events, and coordinate CHW participation and staffing.
• Use outreach and event data to identify gaps and inform continuous improvement in engagement strategies.
• Coordinate outreach-related communications, including flyers, fact sheets, event promotions, and community-facing email and text campaigns, ensuring messages are culturally and linguistically appropriate.
• Work with leadership and communications staff/consultants to align outreach messaging with Equal Hope's broader communications and brand strategy (website, social media, newsletters, PSAs).
• Support CHWs and program staff in using talking points, scripts, and educational materials that are consistent, accurate, and health literate.
• Track and report on outreach communications metrics (e.g., event attendance, sign-ups, referrals generated, reach of PSAs and social posts)
• Plan, coordinate, and execute community-facing events (e.g., health fairs, educational workshops, screenings, celebrations, and campaign activations) that promote Equal Hope programs and services.
• Coordinate event logistics, including site selection, partner roles, staffing, promotion, materials, and follow-up with participants.
• Collaborate with development, communications, and program teams on events that serve dual purposes (education, outreach, advocacy, and/or fundraising).
• Capture and share stories, photos, and key learnings from events to inform future planning and communications.
• Work with the data/evaluation team to ensure CHW and outreach data are collected, cleaned, and available for grant reports, dashboards, and internal decision-making.
• Contribute to grant reports and proposals by providing outreach and CHW activity summaries, success stories, and outcome data.
• Participate in internal planning meetings to ensure CHW, outreach, and communications activities are coordinated with navigation, clinical partners, and other programs.
