Description:The role of the Medical Social Worker is to work with the interdisciplinary team to ensure a safe discharge of all adult patients. Duties include but are not limited to:
• Identifies patients with barriers to a safe discharge plan.
• Completes psychosocial assessments on all patients.
• Documents all clinical assessments, notes, and referrals in Epic.
• Completes screens and coordinates all new skilled nursing facility and rehab facility referrals.
• Assesses all cases involving child abuse/neglect, domestic violence, elderly abuse, and sexual assault.
• Educates patient/family about post-acute care options and the role of choice.
• Educates patient/family about managed care and payor issues
This is a full-time day shift position 9:00am-5:00pm Monday-Friday including every 4th weekend, both Saturday and Sunday, located at NewYork-Presbyterian Brooklyn Methodist Hospital.Preferred Criteria- At least one year of experience in an acute care setting
- Discharge planning experience
- Experience with Epic and Allscripts
Requirements:- Master's Degree in Social Work (MSW)
- New York State License as a Licensed Masters Social Worker (LMSW).
- Communication and negotiation skills
- Ability to work independently and collaboratively with the interdisciplinary team.
- Work efficiently and effectively under pressure.
- Work comfortably within the limitations of the medical setting.
- Computer literacy.

Salary Range:$44.16-$53.81/Hourly
