Salary: $73,000 - $95,000

The Manager of Resident Navigation serves as the primary advocate, resource, and care coordinator for residents and their families within a senior living community. This role focuses on helping residents achieve and maintain their highest level of health, wellness, independence, and quality of life by providing guidance, support, and access to services across the continuum of care.

The position leads resident health and wellness initiatives using the 8 Dimensions of Wellness framework and works collaboratively with residents, families, physicians, community leadership, and interdisciplinary teams to coordinate care, support transitions between levels of care, reduce barriers to wellness, and improve resident satisfaction and retention.

Key Responsibilities
  • Build trusted relationships with residents and families.
  • Coordinate care, services, and resources to support residents' health and wellness goals.
  • Lead and facilitate resident transitions among Independent Living, Assisted Living, Memory Care, and skilled nursing services.
  • Chair and coordinate interdisciplinary Collaborative Care Team meetings.
  • Serve as a resident advocate and liaison with families, healthcare providers, and community staff.
  • Support hospital discharges, care transitions, recovery planning, and risk reduction initiatives.
  • Develop and oversee wellness programs, educational events, support groups, and community resources.
  • Manage emergency preparedness planning and resident safety initiatives.
  • Promote resident engagement, community participation, and retention.
  • Track outcomes, maintain required documentation, and contribute to operational and occupancy goals.

  • Strong experience in senior living, healthcare, care coordination, or case management.
  • Exceptional relationship-building and communication skills.

  • Experience collaborating with interdisciplinary teams.
  • Strong organizational, leadership, problem-solving, and project management skills.
  • Knowledge of aging services, wellness programming, healthcare systems, and resident advocacy.


Preferred Qualifications
  • 3 to 5+ years of experience in senior living, healthcare, care management, or related fields.
  • Bachelor's degree in Social Work, LSW, MSW, LCSW or RN or LPN.
  • Experience with care transitions, resident wellness programs, and interdisciplinary care planning.
  • Experience working with the senior population.
