Department & Position Overview: The Manager of Care Management will assist with building a Health Home Care Management Program from the ground up by maintaining relationships with Health Home Leads; screening patients for the program, enrolling patients in the program; onboarding, training, and supervising a team; reviewing billing, and ensuring NY State and lead Health Home compliance regulations are upheld.
Every Day You Will: - Collaborate with the VP of Operations and Team Lead to continue building the care management program
- Develop workflows and processes to operationalize all policies & procedures
- Build a team of care management staff, including interviewing, hiring, training, planning and assigning work, and providing performance feedback
- Review assessments and uphold care management policies and procedures, ensuring all care management staff are appropriately documenting all encounters, assessments, care plans and other Health Home requirements
- Work with the VP of Operations and Health Home leads to establish and achieve quality improvement, quality assurance, and compliance goals and targets
- Ensure compliance with all Health Home standards and regulations, including chart audits, training and re-training with staff, and working with Health Home leads to address any issues
- Provide guidance to care management staff on crisis intervention, including ensuring proper incident reporting
- Provide case coverage and de-escalation as needed, especially during staffing gaps
- Provide guidance during patient crisis situations, monitor and track these incidents, and ensure proper incident reporting takes place
- Continuously conduct qualitative audits of all documented patient and collateral interactions, including phone calls, SMSs, emails, in-person visits, virtual visits, etc.
- Conduct ongoing chart audits in Care Management platform to ensure all patient charts are in compliance with Health Home NYS and lead Health Home regulations
- Review and sign off on patient assessments within Uniform Assessment System for New York (UAS-NY)
Ideal Candidate Will Possess: - Master's Degree or a Professional Occupational and Healthcare License such as a Registered Nurse License
- LMSW or MSW with prior management experience in a behavioral or healthcare setting
- Care management experience with at least 3 years of strong supervisory experience in the Health Homes field
- Ability to travel in the field to accompany patients to appointments and meet patients in person while providing coverage, if needed
- Ability to travel in the field when needed in escalated situations
- Strong working knowledge of local community resources
- Demonstrated ability to work with data reporting, documentation, compliance, and outcomes
- Strong communication and assessment skills; the ability to relate to patients, their families, and community care providers, along with ability to handle rapidly changing crisis situations
- Able to express empathy and compassion for the underserved
- Experience navigating several data management systems, such as Netsmart, Foothold, and Salesforce
- Demonstrated ability to work with data reporting, documentation, compliance, and outcomes
- Strong communication and assessment skills; the ability to relate to patients, their families, and community care providers, along with ability to handle rapidly changing crisis situations
- Able to express empathy and compassion for the underserved
- Experience navigating several data management systems, such as Netsmart, Foothold, and Salesforce
Nice-to-Haves: - Bilingual Spanish preferred
- 5 + years of experience as Care Manager for the Health Homes program in NYS
At FreedomCare, base pay is one part of our total compensation package and is determined within a range. This provides our employees with the opportunity to professionally grow and develop within a role. The base pay range for this role is between $100,000 and $130,000 per year at the commencement of your first year of employment. Compensation decisions are dependent upon a variety of factors which may include, but are not limited to: skill set, years of relevant experience, education, location, and licensure/certifications.
