Scope of Service
We provide intermittent quality care for homebound patients, of all ages, by offering a network of services to meet the emotional and physical needs of both patients and families. We provide Skilled Nursing, Physical Therapy, Occupational Therapy, Speech Therapy, as well as Certified Home Health Aide care, and Medical Social Worker services. Our service area includes Hilo, Hamakua, North and South Kohala, North Kona, Keaau and Volcano.
Responsibilities
I. JOB SUMMARY /RESPONSIBILITIES
• Manages, administers, and directs all activities of Kohala Home Health (KHH) within established policies of Queen's North Hawaii Community Hospital (QNHCH).
• Ensures services and all care to the patients are provided in accordance with patient and family goals, professional, legal, regulatory, accrediting and administrative requirements.
• Collaborates with the community, patients, families, problem-solves, makes sound judgment, embodies the mission, vision and values of Queen's North Hawaii Community Hospital and The Queen's Health Systems.
II. TYPICAL PHYSICAL DEMANDS:
• Essential: finger dexterity, seeing, hearing, speaking.
• Continuous: sitting, static gripping of an object for prolonged periods.
• Frequent: walking.
• Occasional: standing, stooping/bending, climbing stairs, walking on uneven ground, lifting and carrying usual weight of 1 pound up to 5 pounds, reaching above, at and below shoulder level, pushing, pulling, frequent gripping of an object.
• Operates devices, equipment, instruments and tools related to professional discipline.
• Operates computer, calculator, telephone, fax, printer and other business office machines.
III. TYPICAL WORKING CONDITIONS:
• Not substantially subjected to adverse environmental conditions.
• Some exposure to hazardous conditions from communicable diseases and/or body fluids; some toxic substances and other conditions common to a homecare environment.
IV. MINIMUM QUALIFICATIONS:
A. EDUCATION/CERTIFICATION AND LICENSURE:
• Current Hawaii State license as a Registered Nurse, Physical Therapist, Occupational Therapist or Speech Language Pathologist.
• Bachelor's degree in a health care related discipline, preferably in Nursing.
• Valid U.S. driver's license, vehicle and vehicle insurance as required by law.
• Meets and complies with qualifications set forth in QNHCH fleet/Motor Vehicle policy.
• Current BLS/HCP certification.
B. EXPERIENCE:
• Minimum two (2) years of experience in a health care leadership/supervisory position.
• Minimum five (5) years clinical experience as Registered Nurse, Physical Therapist, Occupational Therapist or Speech Language Pathologist.
• Experience to demonstrate excellent oral and written communication skills.
• Experience in a home health setting preferred.
