The Manager, Care Coaching serves as the primary point of contact for Care Coaches and is responsible for maintaining team compliance. You will also collaborate with other departments. It is our priority to maintain members in the least restrictive environment. In addition to improving member's health and well-being.
The Manager, Care Coaching employs a variety of strategies, approaches and techniques to manage a member's health issues. In this role you will support a team of 15-17 Care Coaches, be accountable to overall team compliance, coaching, and accountability models. You will support metric driven reports and identify barriers and supports to overcome them. You will maintain caseload ratios, while minimizing mileage and overtime.
Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through use of assessment, data, conversations with member, and active care planning. Decisions are typically related to resources, approach, and tactical operations for projects and initiatives involving own departmental area. Requires cross departmental collaboration, and conducts briefings and area meetings; maintains frequent contact with other managers across the department.
Required Qualifications- Bachelor's Degree in a health-related field
- 2+ years of managed care experience in Medicare and/or Medicaid.
- 1+ years of Quality/Compliance experience
- Proficiency in analyzing and interpreting trends
- Comprehensive knowledge of Microsoft Word, Excel and Access
- Progressive experience in the health solutions industry, with emphasis with leading and managing teams
- Proving track record of leading and motivating teams
- Must not exceed 3 hours driving time to the region/s they are leading (Jacksonville, Daytona Beach, Deland).
Preferred Qualifications- Proficiency in analyzing and interpreting financial trends
- Previous experience in utilization management, discharge planning and/or home health or rehabilitation
- Experience with health promotion, coaching and wellness
- Health Plan experience
- 1+ years of management/leadership experience.

Pay RangeThe compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
$78,400 - $107,800 per year
This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
