Client is seeking an experienced
Manager Care Management - Social Work to oversee daily operations and staff management within the Care Management Department.
The Manager is responsible for leadership and coordination of care coordination, case management, discharge planning, utilization management, social work services, and pre-admission case management functions. The role serves as a liaison between patients, families, physicians, healthcare teams, managed care organizations, skilled nursing facilities, and community agencies to ensure high-quality patient-centered care and operational excellence.
This leadership position is accountable for implementing best practices, ensuring compliance with clinical standards, improving patient outcomes, and maintaining the financial integrity of the organization.
Key Responsibilities- Oversee daily operations of the Care Management Department
- Manage care coordination, discharge planning, utilization review, and social work services
- Supervise and mentor case managers and social work staff
- Collaborate with physicians, nurses, patients, families, and community agencies
- Ensure compliance with clinical, regulatory, and organizational standards
- Implement and monitor best practices for patient care and utilization management
- Support financial stewardship and operational efficiency
- Coordinate with managed care organizations and skilled nursing facilities
- Lead quality improvement and patient outcome initiatives
- Manage staffing, scheduling, employee engagement, and performance management
- Support interdisciplinary collaboration across hospital departments
Required QualificationsEducation- Master's Degree in Social Work (MSW) from an accredited school required
Licensure- Washington Clinical Independent Social Worker License (LICSW) required
Required Experience- Minimum 6 years of Case Management experience in an acute care setting
- Minimum 2 years Managerial/Director-level experience, preferably in Case Management within a large hospital
- Leadership experience managing care coordination and social work teams
- Experience with utilization management and discharge planning
Preferred Experience- Large hospital or regional medical center experience
- Experience leading multidisciplinary care management teams
- Strong knowledge of acute care case management workflows
- Experience with managed care and payer coordination
- Quality improvement and operational leadership experience
