LPN for Developmental Disabilities Support

Location: Philadelphia Area

Shifts: 8:00 AM - 8:00 PM | 8:00 PM - 8:00 AM

Job Type: Full-Time | Part-Time | Per Diem

Pay: $40 p/hr

Isoke Developmental Services is seeking experienced, dependable Licensed Practical Nurses (LPNs) to provide skilled care for a medically complex individual in a 2:1 home-care setting alongside a Direct Support Professional.

This is a meaningful assignment for a nurse who is confident with tracheostomy care, ventilator support, G-tube care, medication administration, and emergency response and values consistent, person-centered care in the home.

Key Responsibilities - Clinical & Behavioral Health Care
  • Deliver skilled nursing services in a 2:1 home and community support setting per individualized service plans (ISP)
  • Provide skilled trach, ventilator, suctioning, respiratory, G-tube care
  • Administer oral, enteral medications, support hygiene and safety needs
  • Implement behavioral support strategies and manage crises using trauma-informed approaches
  • Monitor and document vital signs, behavioral patterns, and health changes through Electronic Visit Verification (EVV)
  • Maintain a safe and structured environment while encouraging independence and person-centered routines


Care Coordination & Documentation
  • Collaborate with physicians, behavior specialists, support coordinators, and families to ensure continuity of care
  • Support DSPs with hands-on training in health protocols, medication support, and behavior management
  • Ensure full compliance with HIPAA and Pennsylvania Chapter 6100 regulations, particularly for in-home and community supports
  • Attend appointments and follow up with updates and reporting through the appropriate documentation systems


Training, Advocacy & Development
  • Participate in regular training in medication administration, G-Tube management, positive behavior support, and crisis prevention
  • Empower families with education on behavioral triggers, medication schedules, and safety best practices


Minimum Requirements
  • Active LPN License in Pennsylvania or Multi-State License (MSL) - Required
  • 5 years of Trach, ventilator, and G-tube experience required
  • Experience in IDD care, behavioral health, or enhanced support services
