How you'll make an impact in this role- Coordinate the interdisciplinary plan of care across the continuum from admission to discharge while integrating patient and family preferences
- Monitor patient care through assessments, evaluation, and record reviews while serving as a resource for documentation, service appropriateness, and post-discharge options
- Advocate for necessary resources and barrier removal while maintaining ongoing dialogue with supervisors and providers to execute care plans
- Provide clinical oversight of other associates as needed
What minimum requirements you'll needLicensure / Certification / Registration:
- One or more of the following required:
- Clinical Counselor obtained prior to hire date or job transfer date.
- Counselor specializing in Clinical Practice obtained prior to hire date or job transfer date.
- Social Worker specializing in Clinical Practice obtained prior to hire date or job transfer date.
- Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.
- Case Manager credentialed from the American Case Management Association (ACMA) preferred.
Education:
- Master's degree in Social Work required.
Work Experience:
- 3 years of experience preferred
What additional preferences we're seeking- Hospital care management experience needed
- Previous Behavioral Health experience desired
- Able to commute, if needed, to the Chicago, IL area
