POSITION: Integrated Behavioral Health Provider

HOURS: 1.0 FTE (80 hours per pay period) Exempt
ANNUAL SALARY: $90,064 - $115,814.40 + Productivity Bonus available
GENERAL RESPONSIBILITIES:The Integrated Behavioral Health (IBH) Licensed Therapist provides behavioral health assessment, diagnosis, treatment, consultation, and care coordination as part of an interdisciplinary healthcare team. This position plays a key role in developing and implementing CCM Health's Integrated Behavioral Health program by collaborating with leadership and clinical staff to establish workflows, improve access to behavioral health services, and promote whole-person, patient-centered care.
REQUIRED EDUCATIONS/TRAINING:- Master's or Doctoral degree in Social Work, Counseling, Marriage and Family Therapy, or Psychology from an accredited institution.
- Current Minnesota independent licensure (LICSW, LPCC, LMFT, or Licensed Psychologist).
- Training in Integrated Behavioral Health, Collaborative Care, Primary Care Behavioral Health, or related model preferred.
- Training in evidence-based therapies including CBT, Motivational Interviewing, Solution-Focused Therapy, and Trauma-Informed Care.
SKILL(S):- Knowledge of diagnostic criteria and evidence-based behavioral health practices.
- Strong clinical assessment, crisis intervention, and treatment planning skills.
- Excellent communication and interdisciplinary collaboration.
- Ability to develop clinical workflows and support program implementation.
- Strong organizational, documentation, and problem-solving skills.
- Proficiency with electronic health records.
EXPERIENCE:- Minimum of two years of post-licensure behavioral health experience preferred.
- Experience providing psychotherapy across lifespan.
- Experience in integrated primary care or collaborative care preferred.
- Experience with program development, quality improvement, or workflow implementation is desirable.
- Experience working with individuals experiencing depression, anxiety, trauma, substance use disorders, chronic illness, and severe mental illness.
