Shady Grove seeks to hire an Inpatient Care Navigator Social Worker for our Care Navigation Team who will embrace our mission to extend God's care through the ministry of physical, mental, and spiritual healing.

As an Inpatient Care Navigator Social Worker, you will:
• Completes comprehensive psychosocial assessment with focus on patient's requirements as they transition to the next level of care
• Collaborates with physicians, nurses, and other disciplines involved with care of the patient to foster a coordinated approach to discharge planning
• Identifies and navigates patient testing and treatment to reduce barriers to patient discharge and preventing delays in patient care; communicates barriers to leadership for resolution and trending
• Communicates with Utilization Review staff on any denials, issues or barriers to discharge
• Identifies services and resources available in the community and assists with patient connection to these services
• Participates in Interdisciplinary Rounds and other patient care conferences
• Documents assessments and interventions according to departmental standards
• Participates in process improvement activities

Qualifications include:
• Bachelor of Social Work (BSW) or Master of Social Work (MSW)
• Minimum of 3 years of experience in diverse clinical setting is required
• Prior case management or discharge planning experience is preferred
• Prior experience with Cerner EMR is preferred
• LBSW, LMSW, LCSW
• Active American Heart Association Basic Life Support (BLS) certification required

Work Schedule:
  • Employment Type: Full-time
  • Hours per Week: 40 hrs/week
  • Typical Daily Schedule: Monday through Friday
  • Shift Type: Days
  • Weekend Requirements: Every other weekend rotation
  • Holiday Requirements: Follow company holiday calendar
  • Shift Differential Eligible: Yes


Pay Range:
$71,932.12 - $107,889.60
If the salary range is listed as $0 or if the position is Per Diem (with a fixed rate), salary discussions will take place during the screening process.

Under the Fair Labor Standards Act (FLSA), this position is classified as:
United States of America (Exempt)
