Position Overview:
Provides clinical and administrative direction to personnel and activities of nursing clinic associated with providing quality patient care. Assists in the management and integration of the activities and resources of the department locations. Identifies implements and executes process improvement initiatives in line with dashboard targets. Monitors, communicates and implements action plans as needed, based on results of employee, clinician and patient satisfaction surveys. Consistently and effectively implements and monitors policies, workflows, competency and training. Provides direct patient care per their license and scope of practice. Less than 50% of time spent on average is performing the duties of the individuals being supervised.

Job Description:

OPERATIONS/DEPARTMENT MANAGEMENT / CLINICAL LEADERSHIP:
• Functions as role model for nursing and support staff with patient care planning/decision making/ follow-up, communication, prioritization, and problem solving.
• Counsels personnel (career mobility, motivation, recognition, problem solving, evaluations, grievances, progressive disciplinary action) and documents in a timely manner.
• Prepares, presents, and submits performance appraisals for designated staff.
• Actively involved in department recruitment, interviewing, reference checks, and offering of positions.
• Orients/precepts and provides instructions/directions for staff/students. Follows-up and assures completion of department orientation and work performance objectives.
• Immediately respond to patient, physician and staff concerns, completes follow-up and reports results to management.
• Conducts meetings with unit staff for information sharing and problem solving.
• Prioritizes projects, workload, clinical demands, and department functions. Keeps management informed of progress.
• Projects staffing needs for the next day as well as a weekly and monthly focus. Collaborates to meet the needs of the department.
• Demonstrates flexibility and the use of innovative methods to achieve required staffing while considering unit budgetary guidelines, including authorization for overtime.
• Demonstrates understanding of development of department operating and capital budgets.
• Reviews, signs, and ensures accuracy of employee time keeper records. Reports issues or concerns to management.

DOCUMENTATION/QUALITY:
• Demonstrates knowledge of patient care guidelines/policy as evidenced by quality of documentation.
• Documentation is accurate, timely, using the electronic health record.
• Assesses and monitors documentation of care team using the electronic health record, to ensure that orders and encounters are completed and closed.
• Documents communications with other members of multidisciplinary team using standardized work flows.
• Works closely with care team to ensure that clinical documentation is in compliance with all applicable regulations.
• Demonstrates proper use of quality assurance (QA) process including investigation and follow-up of pertinent information.
• Actively participates in unit-based, divisional and departmental committees to promote change and establish standards of practice in compliance with regulatory agencies.

SAFETY/COMPLIANCE:
• Demonstrates safety, proficiency and knowledge of technical functions required in the accomplished of regular patient care activities, including:
• All types of patient care equipment including oxygen/respiratory delivery devices.
• Medication administration.
• IV management, including administration of blood/blood products.
• Specimen collection, including identification of patient, confirmation of correct requisitions, timing of collections, correct procedure for collection, and timely delivery to lab.
• Emergency responses.
• Infection control issues, including sterile technique and universal precautions.
• Mobility/immobility issues.
• Contributes to clean, safe, and orderly condition of all unit areas.

PATIENT CARE PROCESS/PATIENT EDUCATION (when needed):
• Demonstrates understanding of pathophysiology and rationale of special needs and common problems associated with physiological,
psychosocial, safety, learning, rehabilitative, spiritual/cultural, and self-care issues.
• Proactively manages patient care to avoid complications and risk management issues.
• Completes thorough assessment in a timely manner.
• Accurately verifies, prioritizes, and implements physician orders and plans for care.
• Accurately interprets assessment findings, reports abnormal data to physicians & team members, and initiates appropriate interventions.
• Develops and implements individualized plan of care based on identified patient needs.
• Evaluates patient's response to plan of care. Reviews and reconciles variances.
• Collaborates with physician and other team members to assure consistency with medical plan.
• Meets regularly with patient and family to update plan of care.
• Communicates plan of care to appropriate departments/facilities/agencies to promote seamless care across the continuum of services.
• Works with clinical staff to identify support systems/coping mechanisms/barriers to learning.
• Ensures that procedures, therapies, and treatments are effectively explained to patient and significant others. Confirms understanding
of it.
• Supports clinical staff to individualize teaching plan and provides information consistent with standard of practices.
• Leads efforts to develop and use of appropriate and effective resources to provide patient/family education based on treatment and
identified needs. Accommodates for learning barriers (e.g., language, hearing, and vision).

EDUCATION:

Graduate of an accredited school of nursing.

ERTIFICATION & LICENSURE:

RN-Registered Nurse of California

BLS-Basic Life Support Healthcare Provider

TYPICAL EXPERIENCE:

5 years of recent relevant experience.

SKILLS AND KNOWLEDGE:

Demonstrate basic nursing skills.

Demonstrate effective English verbal and written communication skills.

Self directed in coordinating the care plan for patient care delivery.

Completion of SMF orientation: Basic Skills Assessment, Epic and IDX course work and competency assessment.

Demonstrate knowledge of community resources, legal policies relating to patient decisions and outcomes.

Demonstrate skills in delegation, leadership and team management concepts.

Demonstrate knowledge of the performance standards of other team members.

Completion of department specific orientation.
