Responsible for providing on-site leadership and support to offices experiencing significant clinical or operational challenges to ensure a return to stability and positive survey outcomes. Assures continuity, integrity and the highest-level quality of health care service. Works in tandem with the area Operational Leaders while also managing a team of Rapid Response RN's who support offices during times of staffing vacancies.

Responsibilities

  1. Manages and directs Rapid Response RN's to assure each assignment achieves the needs of the care center as directed by the AVPO and local DOO/CM.
  2. Develops and directs the implementation of corrective action plans in care centers as a result of assessment and in response to unfavorable internal or external survey outcomes.
  3. Directs and participates with departments and care center management in providing feedback and education for the development and implementation of policies, processes and procedures, and systems to ensure compliance and positive survey outcomes.
  4. Drives improvement in care center write-offs via on-site visit to assess current state, develop action plan, implement, and monitor for improvement.
  5. Provides onsite education when applicable due to process gaps including, but not limited to, creating action plans with the Director of Operations to implement and monitor for improvement. Process gap examples (care center with low % of Welcome home modules completed, referral mgt, high live dc, high CPD metrics, increased turnover, high salary costs but productivity issues, etc.)
  6. Assists offices in managing multiple action plans, providing direction and support in prioritization of action items and integration of the action plans.
  7. Ensures compliance with local, state and federal regulations and accreditation standards. Provides support, direction, and feedback to staff during the clinical compliance process.
  8. Directs/delegates staff assignments, assigns teams, and directs documentation activities conducting education as required for particular recovery assistance.
  9. Assists location with interpreting standards and company policies and procedures to ensure compliance with external regulatory authorities and ensures that caregiver clinical documentation meets internal standards.
  10. Provides guidance and interpretation to care center management and staff on matters affecting the delivery of service to patients. May oversee and support clinical management orientation and training in the care center to ensure the orientation process is completed.
  11. Provides expertise to operational staff in the office, as well as Regional Leadership Team (RLT), regarding follow-up action for correction of deficiencies and identification of operational issues. Directs and monitors resolution of identified deficiencies in conditional level care centers on behalf of the governing body to ensure ongoing compliance and makes written/verbal reports to the governing body and its committees as requested.
  12. Monitors and evaluates employee performance. Holds employees accountable for the attainment of defined goals and objectives. Executes effective performance redirection when needed. Develops employees through coaching, mentoring, and formal/on the job training and development opportunities.
  13. Performs other duties as assigned.

Qualifications

Required
  • Current, unencumbered license to practice as a Registered Nurse.
  • Five (5+) years' hospice experience.
  • Three (3+) year's management experience.
  • Proven track record in performance management and effective leadership of direct reports.
  • Knowledge of updated coding compliance guidelines, coding clinic references and CMS regulations.
  • Knowledge of care delivery documentation systems and related medical record documents.

Preferred
  • Strong knowledge of Conditions of Participation for Hospice and state regulatory bodies.
