Full Job Description
Position Description: Hospital at Home (H[redacted]) Acute Care/MACU (mobile acute care unit) RN Team Leader is responsible for assisting with reviewing patients in EMR with H[redacted] case manager and social worker(hospital personnel) for program eligibility. MACU RN Team Leader provides coordination of homecare services, care, and documentation of care for program patients in accordance with Quality Standards and under the guidance of a clinical supervisor.

Performance Responsibilities and Standards:
  • Ensure quality and safe delivery of acute hospital care at home services.
    • Assist with screening of hospital patients for program eligibility
    • Scheduling patients to MACU nurses and therapists.
    • Educate patients and families about program, expectations, etc.
    • Facilitate communication among departments, agencies, and an interdisciplinary team of clinicians.
    • Coordinate internal and external resources to meet patient needs.
  • Provide clinical care as needed to assigned patients in the private home environment.
    • Patients require a minimum of 2 nursing visits per day, use of telehealth monitoring and virtual visits, joint visits with medical providers as needed.
    • Collaborate with medical providers at hospitals, communicate any changes in patient condition, routine clinical updates, and progress toward goals.
    • Administer medications and care to patients as ordered.
    • Maintain skill competency as required for delivery of quality complex care including proficient assessment skills, high-tech IV skills, procedural skills and equipment use (i.e. catheter care, drainage tubes, oxygen, durable medical equipment, Telemonitoring, etc.)
  • Perform all other duties as assigned.

Requirements:
  • Current license as a Registered Nurse in New York State.
  • Bachelor's degree is preferred from an approved school, acceptable to the New York State Department of Health, preferred.
  • Experience as a clinician with home and community-based services preferred.
  • Ability to provide care to individuals in private homes.
  • Strong written and verbal skills with ability to manage stressful situations including conflict resolution.
  • Excellent computer knowledge with proficiency in Microsoft Office, or other word processing, spreadsheet, and database management
  • Excellent organizational skills and ability to prioritize multiple tasks.
  • Efficient and flexible in problem solving with ability to motivate and educate others.

Physical Requirements:
  • Speaking/visual/hearing ability sufficient to comprehend and express written/verbal communication.
  • Ability to perform tasks involving physical activity including:
    • Pushing/pulling typical weight 50 pounds, maximum weight 150 pounds
    • Lifting/ carrying typical weight 25 pounds, maximum weight 40 pounds
    • Patient transfers, frequent bending, standing, kneeling, walking in various weather conditions e.g., snow, rain, sleet, and going up and down stairs.
  • Ability to drive-in all-weather conditions throughout the working year.
  • Bloodborne Pathogens Exposure Category I - Employee has tasks that routinely involve exposure or potential exposure to blood, body fluids or tissues)
