Overview

AccentCare is seeking a dedicated and collaborative Hospice Team Physician to provide clinical leadership and medical expertise as part of an interdisciplinary hospice care team. This physician will play a vital role in supporting patients with terminal illnesses, partnering with attending physicians, and ensuring the delivery of high-quality, patient-centered hospice care.

This position offers the opportunity to work closely with nurses, social workers, chaplains, and other healthcare professionals while making a meaningful impact on patients and families during critical moments in their healthcare journey.

Key Responsibilities:
  • Serve as a clinical leader within the Interdisciplinary Group (IDG), contributing to care planning, team discussions, and quality improvement efforts.
  • Provide medical care and guidance for hospice patients, including symptom management, medication review, and support
  • Participate in developing, reviewing, and updating individualized Plans of Care to ensure patient and family needs are met.
  • Complete required certifications, recertifications, and face-to-face encounter attestations in accordance with hospice regulations.
  • Collaborate with Attending Physicians, referring providers, patients, and families regarding care plans, prognosis, and medical decisions.
  • Participate in on-call coverage to ensure continuous (24/7) physician availability and provide timely clinical support.
  • Support appropriate hospice admissions, recertifications, revocations, and discharges.
  • Maintain accurate, timely, and compliant medical documentation.


If you meet these qualifications, we would love to meet you:
  • Must be a licensed Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) in the state(s) of practice, with training or experience in caring for chronically or terminally ill patients
  • Maintains active, unrestricted medical and DEA licenses in the state of Indiana
  • Strong understanding of primary medical care principles.
  • Working knowledge of hospice and palliative care, including pain and symptom management.
  • Familiarity with hospice regulations and Medicare requirements.
  • Strong clinical judgment and decision-making skills.
  • Ability to collaborate effectively with Attending Physicians and interdisciplinary teams.
  • Excellent communication skills for interaction with patients, families, and providers.
  • Ability to manage complex clinical and ethical situations in end-of-life care.
  • (Preferred) Board certification in Hospice and Palliative Medicine
  • (Preferred) Five or more years of clinical experience
  • (Preferred) Oversight of clinical program(s)
