Active RN license in Washington or a compact state
Basic Life Support (BLS) certification from AHA
Current driver's license with reliable transportation
Critical thinking and strong problem-solving skills
Experience in hospice care preferred, but not mandatory
Responsibilities
Conduct thorough in-home assessments of patients
Manage comfort levels, symptom control, and pain relief
Create and update patient care plans per physician directives
Provide skilled nursing care with compassion in home settings
Ensure accurate and timely documentation of care
Engage in quality improvement programs (QAPI)
Collaborate with a diverse healthcare team for holistic support
