Responsibilities

  • Complete comprehensive assessments of patients and families to determine hospice needs
  • Develop and implement individualized care plans, revising them as needed
  • Provide skilled nursing care, administer medications, and manage symptoms
  • Educate patients and families on care techniques, medications, and comfort measures
  • Maintain accurate clinical documentation and communicate changes to physicians
  • Participate in interdisciplinary team meetings and quality improvement initiatives
  • Provide emotional support and counseling to patients and families
  • Supervise hospice aides and delegate responsibilities appropriately
  • Collaborate with an interdisciplinary team committed to excellence
  • Participate in holiday on-call rotation as needed

*Pay Range is $80,000.00 - $85,000.00 per year (depending on experience), plus mileage reimbursement. What You’ll Do Make a meaningful impact every day. As a Hospice RN, you’ll act as a trusted Case Manager, delivering care that supports patients in their homes and communities. You will guide families through care decisions, while providing comfort, dignity, and compassionate support during life’s most tender moments.
