Hospice RN Case Manager - $85,000-$100,000 | Mon-Fri | ~15-Patient Caseload
Location: Bucks and Montgomery County
Schedule: Monday-Friday | 8:00 AM-5:00 PM
On-Call: Rotating | Limited occasional evenings/weekends
Caseload: Approximately 15 patients (subject to change based on census and acuity)
You are not expected to carry the workload alone.
Your Role
As an RN Case Manager, you are the clinical lead for your assigned patients - supported by a strong interdisciplinary team.
You will:
• Conduct comprehensive nursing assessments (with Admissions Nurse support)
• Develop and manage individualized plans of care
• Collaborate with physicians, LPNs, social workers, chaplains & aides
• Educate families on symptom management and disease progression
• Participate in IDT meetings
• Maintain timely, compliant documentation

Qualifications
• Active Pennsylvania RN License
• Hospice or home health experience strongly preferred
• Strong clinical judgment & communication skills
• Valid driver's license & reliable transportation
