Responsibilities

  • Assist in planning and implementing patient care services for Hospice patients.
  • Coordinate documentation related to patient care services.
  • Support families of Hospice patients through care delivery.
  • Ensure compliance with care standards and best practices.
  • Collaborate with interdisciplinary teams to enhance patient services.

Requirements

Assists in the planning, implementation, documentation and coordination of patient care services to Hospice patients and their families.

Region : Bronx

Hours: 9am - 5pm

Salary: $113,576.52 - $140,076.61/annually

Qualifications:
  • Current New York State Registered Nurse License/Permit.
  • Two (2) years experience in nursing which includes any combination of medical-surgical, oncological, palliative and community home health/hospice.
  • At least one (1) year of community hospice or home health experience is preferred.
  • Hospice experience preferred.
  • NYS driver's license preferred.

Education:
  • BSN from an accredited school is Preferred.
