  • Provide community home visits for acute medical management

  • Provide after-hours, weekend, and holiday on-call coverage
  • Participate in the hospice's Quality Assessment and Performance Improvement (QAPI) program
  • Support compliance with all federal, state, and local regulations

Hospice and Palliative Physician

Brevard County, Florida

About the Role

As a core member of our interdisciplinary team (IDT), you will direct the medical component of patient care, validate prognosis and eligibility, and partner closely with patients' primary physicians to ensure seamless, high-quality hospice care. You'll provide clinical leadership, mentorship, and guidance to staff, contractors, and volunteers - helping ensure every patient and family receives compassionate, dignified care during one of life's most difficult chapters.

What You'll Do
  • Evaluate new hospice admissions and assess eligibility, including comorbidities affecting prognosis
  • Round on and supervise inpatient hospice patients, including weekend coverage as needed
  • Lead and participate in Interdisciplinary Group (IDG) meetings, overseeing comprehensive plans of care
  • Prescribe and oversee pharmacotherapeutics in collaboration with case managers, pharmacists, and patients' primary care providers
  • Conduct Face-to-Face visits to confirm ongoing hospice eligibility
  • Provide community home visits for acute medical management, hospice follow-up, eligibility redetermination, and revocation follow-up
  • Complete required documentation, billing, and death certificates within Florida's 72-hour statutory requirement
  • Provide after-hours, weekend, and holiday on-call coverage
  • Participate in the hospice's Quality Assessment and Performance Improvement (QAPI) program
  • Support compliance with all federal, state, and local regulations

What You Bring
  • Graduate of an accredited medical school or school of osteopathy
  • Board Certification in Hospice and Palliative Medicine (HPM) or Hospice Medical Director Certification (HMDC) preferred
  • Active, unrestricted Florida medical license (Chapter 458 or 459) with hospital admitting privileges in the service area
  • Active Medical and DEA license
