Qualifications

• Graduate from an accredited nursing school; Bachelor's degree preferred.
• Current Registered Nurse state license.
• At least 1 year of recent acute care experience in an institutional setting.
• Minimum 1 year of recent home care experience, preferably in hospice.
• Valid driver's license and reliable, insured transportation.

Responsibilities

• Initiate and revise patient care plans as needed based on patient status and needs.
• Determine nursing diagnoses using health assessment data.
• Develop care plans that set goals, incorporating palliative nursing actions and involving patients and families.
• Provide healthcare instructions to patients according to assessments and care plans.
• Assist patients with daily living activities to promote self-sufficiency and comfort care.
• Act as a Case Manager for assigned caseload when designated by Clinical Manager.
• Communicate with physicians about patient needs and changes in condition; obtain necessary orders.
• Teach patients and families about self-care techniques and provide health counseling opportunities.
