Qualifications

• Valid state RN license required.
• At least 1 year of recent acute care experience in a hospital setting.
• Basic Life Support (BLS) certification and hands-on CPR training.
• Must possess a valid driver's license and auto insurance.
• Associate's or Bachelor's degree in Nursing, with a preference for 1 year of home health experience.

Responsibilities

• Assess patients and their families to identify home care requirements.
• Conduct thorough physical assessments and document medical history.
• Create and update personalized care plans in collaboration with patients and families.
• Identify discharge planning needs to ensure readiness before patient discharge.
• Provide counseling and support for nursing and health-related issues.
