Qualifications

• Licensed healthcare professional (physician, physical therapist, speech language pathologist, occupational therapist, or registered nurse)
• Familiarity with home health and hospice agency operations
• Minimum of three years' experience in home health services within the last five years
• Knowledge of Medicare, Medicaid, and third party payer requirements
• Understanding of OASIS data collection and reporting process

Responsibilities

• Oversee patient and personnel assignments
• Coordinate and review patient care and referrals
• Ensure continuous assessment of patient needs
• Develop and update individualized care plans
• Manage staffing levels and conduct performance evaluations
• Facilitate meetings and review care reports with team members
• Maintain compliance with regulatory standards and communicate updates
