Full Job Description
Schedule: Full-time| Monday - Friday | 8:00 a.m. to 4:30 p.m.

Pay Range: $72,000 - $90,000 (salaried exempt), based on experience, qualifications, and location. This position also includes unlimited incentive bonuses.

Territory: Park Rapids, Fergus Falls & Wadena

As a Home Health Liaison, you are the bridge between compassionate care and those who need it most. You'll be the trusted face of Vivie Home Health to hospitals, skilled nursing facilities, physicians, and the broader community-helping individuals and families navigate end-of-life decisions with clarity and care. Your mission: ensure smooth transitions, educate on home health services, and strengthen relationships that expand access to quality, patient-centered care.

As a Home Health Liaison, you will:
  • Build & Nurture Referral Relationships - Develop and maintain strong partnerships with physicians, discharge planners, skilled nursing facilities, and community organizations to support timely and appropriate home health referrals.
  • Educate & Advocate - Promote home health philosophy through compassionate education to referral sources, community groups, and the general public. Serve as a knowledgeable guide for families.
  • Support Admissions & Transitions - Collaborate with clinical teams and intake staff to assess patient eligibility and ensure a smooth, dignified transition into services.
  • Lead Community Engagement - Represent Vivie Home Health at community events, professional gatherings, and health fairs. Share resources and build brand awareness.
  • Partner with marketing to refine outreach tactics and materials that resonate with the communities we serve.
  • Analyze & Report - Monitor referral activity and trends. Provide regular reports and feedback to leadership to support growth and quality outcomes.
