What We Offer! • Competitive salary + Full benefits package + Generous PTO • 401(k) plan with company matching options • Up to a $1,500 CEU stipend per year, plus all expenses paid for any specialty certifications towards Schroth, Pelvic Floor, OCS, PCS, NGS or WCS • Opportunities for professional growth and development • A professional, supportive and fun work environment fostered by a team of well-respected clinicians who take pride in the challenges of meaningful work • Use of modern AI documentation tools for easier note taking! • Focus on Patient Care - Quick EMR (No Oasis required) • Great work/life balance • Flexibility and Autonomy - see patients any 7 days of the week • Preference given to see patients in your primary geographic area Responsibilities: • Accept and treat patients within your designated geographic area • Evaluate patients' needs for physical therapy in the home setting and develop individualized treatment plans • Develop a plan of care and treatment goals in consultation with the physician, patient, and family • Plan, prepare and implement customized therapy programs to restore function and promote independence • Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques • Provide timely and accurate documentation for each visit • Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning • Deliver care in compliance with agency policies and professional practice standards Qualifications • Physical Therapist Program Graduate • Licensed Physical Therapist in the state of New York or eligible. • Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance. • Ability to travel to patients' homes within the assigned geographic territory Additional Information The anticipated base salary range for this position is $95,000.00-$110,000.00. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography. Medical, dental, vision, 401(k), paid time off, and other benefits are also available, subject to the terms of the Company's plan.
