Location: Broome County, New York

Salary: $85,000 per year
Position: Full-Time Home Care Field RN

As a Home Care Field RN, you will travel to client homes and community-based settings throughout the assigned service area to conduct nursing assessments, complete aide supervisions, develop Plans of Care, and help ensure clients receive safe and personalized care.

Home Care RN Responsibilities
  • Travel to client homes and community-based settings throughout Broome County
  • Conduct comprehensive in-home nursing assessments
  • Perform head-to-toe evaluations, including medication and diagnosis reviews
  • Complete required aide supervisions
  • Provide clinical guidance and support to home health aides and caregivers
  • Develop, review, and update individualized Plans of Care (POC)
  • Orient aides to each client's Plan of Care and specific care needs
  • Monitor clients for changes in condition and report concerns appropriately
  • Communicate with clients, families, caregivers, aides, and office staff
  • Complete accurate and timely clinical documentation
  • Maintain compliance with applicable healthcare regulations and company policies
  • Help ensure clients receive appropriate, safe, and high-quality home care services
Registered Nurse Requirements
  • Current and active New York State Registered Nurse (RN) license
  • Ability to travel throughout Broome County and assigned service areas
  • Medical Professional Liability insurance
  • Strong nursing assessment and clinical documentation skills
  • Comfortable using Gmail and Microsoft Word/Excel
  • Excellent communication and interpersonal skills
  • Ability to manage a field-based schedule and work independently
  • Professional, reliable, organized, and compassionate
  • Must meet all company hiring and onboarding requirements
Preferred Qualifications
  • Previous home care, home health, community health, or outpatient nursing experience
  • Familiarity with Medicaid, Medicare, or MLTC plans
  • Experience developing or updating Plans of Care
  • Experience conducting patient assessments and aide supervisions
  • Bilingual in Spanish and English
  • Strong clinical assessment and care coordination skills
  • Comfortable working with clients with a variety of healthcare needs and backgrounds
