Overview

Position Overview:

The Hospice Nurse Practitioner is responsible for performing face to face encounters with each hospice patient prior to the start of the 3rd benefit period and all subsequent re-certifications for the purpose of determining continuing eligibility for hospice care.

Productivity Expectation: 10-20 visits per month in Wilson County

Essential Job Functions:
  • Performs the face to face encounter within 30 calendar days prior to start of the 3rd benefit period and all subsequent re-cert periods as scheduled by the appropriate member of the administrative staff.
  • Gathers and documents clinical findings to determine continued hospice eligibility.
  • Shares the clinical findings with the hospice medical director.
  • Attests in writing that he/she performed the face to face encounter and that these findings were provided to the certifying physician.
  • Communicates with Clinical Director and RN Case Manager prior to visits as to pertinent details of patient's disease process and outcome of the actual face to face visit.
  • May make patient consults and visits as appropriate and as requested.
  • May attend interdisciplinary team meetings as appropriate.
  • Provides inservice education as requested
  • Maintains sound privacy and security practices and prevents privacy or security breaches. If breach occurs, takes corrective action.

Requirements:
  • Able to tolerate being physically active the entire working day.
  • Able to transfer, lift and guard patients safely with appropriate level of assistance and without any likelihood of injury to the patient or self.
  • Able to bend, stoop, squat and lift numerous times daily.
  • Able to carry out fine motor and manual dexterity skills required to perform treatments/care for the assigned caseload.
  • Mental acuity and judgment skills to adequately and safely perform job requirements and assure patient safety.
  • Able to see and hear adequately in order to respond to auditory and visual needs and clinical indications of the assigned caseload.
  • No medical problems that could endanger self or others if such should occur while providing patient care.
  • Must be able to speak, write, read and understand English
  • Must be able to travel
  • Must be able to lift 50 pounds
  • Prolonged walking, standing, bending, kneeling, reaching, twisting
  • Must be able to sit and climb stairs
  • Must have visual and hearing acuity
  • Must have strong sense of smell and touch
  • Must be able to sufficiently reposition patients and move equipment without assistance
  • Must be able to appropriately respond physically and mentally to emergency situations in the home or during transport.
  • Must be able to function in a wide variety of environments which may involve exposure to allergens and other various conditions
  • Possible exposure to blood, bodily fluids and infectious diseases.

Preferred:
  • Current License as Nurse Practitioner in the State of Tennessee
  • Minimum of one (1) year medical/surgical experience. Hospice/Home Health experience preferred.
  • Acceptance and support of the hospice concept of care.
  • Demonstrated ability to assess and respond to the needs of patients and families in a variety of settings.
  • Well organized with excellent time management skills, excellent assessment and problem solving skills; excellent communication skills.
  • An understanding of Medicare regulations, and Joint Commission standards governing the delivery of hospice services.
  • Current, valid Tennessee Driver's License.
  • Proof of automobile insurance coverage.
