Program: Community Network of Specialized Care (CNSC)Status: Full-time, Union, PermanentHours: 36.25/weekSalary: $93,492 - $118,487 annually Date Posted: September 11, 2026Closing Date: September 25,2026Position Description: Reporting to the Manager, CNSC, the Health Care Facilitator's (HCF) primary focus is on offering nursing service to people with intellectual/developmental disabilities (IDD) including capacity building and bridging developmental disabilities services system with health care and other systems. The HCF will provide care planning and consultation regarding health supports of the intensive supports needed for adults with developmental disabilities who have complex medical supports need. They provide support to the person with IDD and circle of care or support partners and assist the client to navigate the provincial healthcare system. This position is a combination of remote and in-person work. Candidates will be expected to deliver in-person services as needed.
Main Responsibilities:- Conduct comprehensive health assessments of adults with developmental disabilities and other specialized needs.
- Support in situations involving complex transfers of care.
- Provide an advocacy role, as required, ensuring that the rights of people with complex and multiple needs are upheld.
- Work with clients, families, internal and external inter-professional teams to offer health care navigation recommendations, and other community resources.
- Participate in care conferences with other providers to ensure coordination of services to the client.
- Offer consultation regarding situations involving RHPA controlled acts and/or health care technology.
- Assess current patterns of care and identify gaps and barriers in services.
- Collaborate with provincial HCFs community of practice regarding cross-regional resources and/or client transitions.
- Collaborate with primary care providers in building capacity or sharing informational support. This includes knowledge of resources and tools, such as the Developmental Disabilities Primary Care Program guidelines and tools.
- Support dissemination of new knowledge through presentation, research, and teaching to varies sectors.
Qualifications: - Master's degree in Nursing from a recognized university.
- Registration in General Class with the College of Nurses of Ontario, in good standing
- Three years of nursing practice experience in a community setting or a combination of community experience in a community or outpatient setting is essential. Relevant hospital nursing an asset.
- Experience working with individuals with developmental disabilities and mental health issues is an asset.
- Experience with case management an asset.
- Thorough current nursing knowledge and proficiency in conducting a comprehensive health history, physical assessment, and a nursing care plan.
- Demonstrated ability to work effectively in a multi-disciplinary environment.
- Demonstrated commitment to and knowledge of community-based health care.
- Good communication and interpersonal skills, with a demonstrated ability to work well with others.
- Second language fluency is an asset.
- Proficient with Microsoft Office 365.
- The successful candidate must provide an acceptable Vulnerable Sector Screening.
