Responsibilities

  • Provide comprehensive primary and preventive care for older adults

  • Perform medication reconciliation and address polypharmacy concerns
  • Collaborate with specialists, behavioral health providers, social workers, pharmacists, and caregivers
  • Support advance care planning and goals-of-care discussions
  • Document patient care accurately in the EHR
  • Participate in quality improvement and community outreach initiatives
  • Supervise Advanced Practice Providers as assigned

Los Angeles, CA | Full-Time | Sign-On Bonus Available

Salary Range: $285,000-$335,000 Annually

About the Role

SJCH is seeking a compassionate Geriatrician to provide comprehensive, patient-centered care to older adults. This role focuses on preventive care, chronic disease management, cognitive assessments, medication management, fall prevention, advance care planning, and coordination of care across multidisciplinary teams.

Key Responsibilities
  • Provide comprehensive primary and preventive care for older adults
  • Manage acute and chronic medical conditions
  • Conduct geriatric, cognitive, and functional assessments
  • Evaluate and manage dementia, depression, frailty, and age-related conditions
  • Perform medication reconciliation and address polypharmacy concerns
  • Collaborate with specialists, behavioral health providers, social workers, pharmacists, and caregivers
  • Support advance care planning and goals-of-care discussions
  • Document patient care accurately in the EHR
  • Participate in quality improvement and community outreach initiatives
  • Supervise Advanced Practice Providers as assigned

Additional Requirements
  • Experience providing care within faith-based or congregational settings, including partnerships with faith communities and religious institutions.
  • Ability to support health education, screenings, wellness monitoring, care coordination, and community-based outreach initiatives.
  • Willingness and ability to conduct home visits and provide care outside of a traditional clinical setting.
  • Demonstrated cultural competency in caring for diverse aging populations, with an understanding of health disparities, social determinants of health, and the needs of underserved elderly communities.
