Pay Range: $40.00 - $48.00 an hour.

PACT MSO has a new Endocrinology office seeking a Full Time RN. The hours are Monday through Thursday 8:00am to 6:30pm. The position requires traveling between Orange, Hamden and Milford office locations and staff follow the provider to each location.
SummaryUnder immediate supervision, supports providers including physicians, nurse practitioners, and physician assistants in delivery of patient care.
Essential Functions- Provides support to assigned Provider, including medication refills, completion of patient forms, patient reminder calls, patient follow up on med changes, lab results, future appointment needs, etc., and liaison between provider and incoming callers outside agencies, providers, patients, etc.
- Assists with providing patient care such as records vital signs, discusses and records chief health concerns, performs tests, and administers injections.
- Assist with triage of patients walking into the clinic to be seen; this entails assessing the patient and determining the priority for which the patient needs to be seen and coordinating the appropriate care based on the assessment.
- Provide phone triage by collecting appropriate information over the telephone to determine the urgency of a health problem and to determine whether medical intervention is needed and how soon treatment should begin.
- Places phone calls to pharmacies to order prescriptions.
- Set-up referral contacts.
- Pulls and prepares patient charts.
- Answers and follows up on patient inquiries.
- Maintains neatness of work area.
- Documents all patient care and completes tasks in EHR.
- Initiate insulin therapy for patients diagnosed with diabetes under the direction of the Physician.
- Educate patients on diabetes management.

- Identify gaps in diabetes knowledge to address and work to fully equip the patient with skills sets needed for self-care.
- Provide full DSMT (Diabetes Self-Management Training) to patients by using digital tools and virtual interactive platforms over a series of visits.
- Support and educate patients on the use of diabetes technology: including proprietary blood glucose meters, commercial continuous glucose monitors and diabetes smartphone apps.
- Independent judgment will be used to identify the need for modification of the current treatment plan based on glucose trends.
- Work with medical provider to explore other interventions or escalate therapy as needed.
- Identify SDOH (Social Determinants of Health) and apply problem solving skills to ensure optimal outcome by coordinating referrals to community resource programs, such as rehabilitation, financial assistance, behavioral health and other social services when needed.
- Serve as a resource to answer any clinical diabetes questions from the team an provide ongoing collaborative diabetes education to internal care team members.
- Using integrated remote patient data, from Glooko for medication adjustment. Use validated titration protocols with support from the medical provider when needed.

Education and Experience- RN Degree from accredited school
- Current State RN license
