ResponsibilitiesThe Field Liaison will be responsible for coordinating services and identifying patients through the health system that may qualify for post hospitalization services. The Field Liaison will assist with identifying patients and assist the hospital with referrals and prior authorization for services aligned with facility standards. The Field Liaison develops relationships and serves as an educational resource for referring providers, patients and family members.The Field Liaison coordinates the care in a timely and efficient manner, identifying and eliminating barriers to care. The Field Liaison is consistent with coordination and assesses the physical, psychological, and social needs of the patient enhancing patient outcomes, increasing satisfaction, and reducing cost of care. The Field Liaison is Responsible for community education and screening as well as monitoring community outreach and promoting survivorship.
QualificationsEDUCATION, EXPERIENCE, TRAINING
1. Current and valid state LVN, PT, OT, RRT or RN License preferred
2. Current BLS certificate upon hire and maintain current.
3. Minimum of five (5) years of healthcare experience preferred.
4. Bachelor27s degree in healthcare or closely related field preferred.

A reasonable compensation estimate for this role, which includes estimated wages, benefits, and other forms of compensation, is $35.50 to $51.00. The exact starting compensation to be offered will be determined at the time of selecting an applicant for hire, in which a wide range of factors will be considered, including but not limited to, skillset, years of applicable experience, education, credentials and licensure.
