What You Will DoThis is an exceptional opportunity for a motivated advanced practice provider to join an employed position within a busy, well-respected practice, supported by a strong referral base and the full resources of a robust health system network.
- Join an established team of 1 physician and 2 APPs
- Call structure: 1 week per month, Monday through Sunday
- Practice setting: Outpatient Primary Care office
- Access to 8 total exam rooms (2 dedicated rooms per provider) utilizing the Athena EMR system
- Employed position within a nonprofit health system
- Board Certified or Board Eligible in Nurse Practitioner
- Eligible for state medical licensure and DEA registration
- Enhance patient access across a rural community, providing exceptional care to a primarily farming-based population
- Drive clinical excellence by performing daily comprehensive chart prep, managing an expected volume of 15 to 20 office patients, and efficiently evaluating lab and imaging results
About the Facility- Focus: Everyday healthcare, preventive screenings, and management of chronic conditions.
- Services: Routine physicals (including sports physicals), preventive health exams, vaccinations (such as flu shots), and lab testing.
- Serving the local city population of Brazil, Indiana, which is approximately 8,350, as well as the surrounding community of Clay County, which has a total population of approximately 26,400

What You Will NeedLicensure / Certification / Registration:
- BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
- Nurse Practitioner specializing in Prescriptive Authority credentialed from the Indiana Board of Nursing obtained within 6 Months (180 days) of hire date or job transfer date required.
- Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date required.
Education:
- Master's degree of Nursing required.
