Schedule: Outpatient | Monday-Friday
What You'll DoComprehensive Primary Care- Provide outpatient primary care services for patients across the lifespan (Family Medicine or Med-Peds scope).
- Diagnose and treat acute and chronic medical conditions.
- Conduct annual wellness visits, preventative screenings, and routine physicals.
- Develop individualized treatment plans focused on long-term health outcomes.
Patient Education & Preventative Care- Counsel patients on preventative health, lifestyle changes, and disease management.
- Promote continuity of care through proactive follow-up and patient engagement.
- Build strong relationships with patients and families within the local community.
Care Coordination & Collaboration- Collaborate with Advanced Practice Providers, Care Managers, Behavioral Health, and specialty teams.
- Coordinate referrals, diagnostics, and follow-up care as needed.
- Participate in a team-based, patient-centered care model.
Clinical Documentation & Compliance- Maintain accurate and timely documentation in the EHR system.
- Ensure compliance with all clinical, regulatory, and quality standards.
- Support quality initiatives and value-based care programs.
What You BringRequired Qualifications- MD or DO from an accredited medical school.
- Board Certified or Board Eligible in:
- Family Medicine or
- Internal Medicine-Pediatrics (Med-Peds)
- Active New Jersey medical license (or ability to obtain prior to start).
- DEA license and CDS registration (or ability to obtain).
- Strong commitment to patient-centered, culturally competent care.
Preferred Qualifications- Experience working in diverse or underserved communities.
- Experience in outpatient primary care settings.
- Value-based care or population health experience a plus.
