Job Description

The Director of Population Health reflects the mission, vision, and values of the organization, adheres to the Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.

Reporting to the Vice President and Chief Medical Office of Population Health, the Director of Population Health provides strategic and operational leadership for system-wide initiatives supporting high-quality care across the continuum. This role is responsible for designing, implementing, and optimizing population health programs that improve clinical outcomes, enhance the patient & clinician experience, and reduce avoidable utilization.

The Director leads ambulatory interdisciplinary teams (Nursing, Social Work, Pharmacy) and collaborates across corporate functions, academic and health network medical groups (primary care & specialty), hospitals, and relevant partners to advance proactive care coordination and disease management.

Responsibilities:

Leadership & Strategy
  • Lead development and optimization of assigned portfolio of population health programs and interdisciplinary teams to support patients across the care continuum
  • Partner with key stakeholders to improve patient experience and outcomes, improve quality, and reduce cost of care
  • Establish strategic plans and leverage data analytics to identify at-risk populations
  • Facilitate continuous exchange of ideas, information, program developments and meaningful cross-departmental engagement and collaboration
  • Demonstrate positive, transformational leadership by supporting colleagues and direct reports through change
  • Data, Analytics & Performance Management
  • Develop and maintain performance monitoring systems, metrics and reporting tools
  • Consistent performance and process improvement
  • Enhance EMR tools for population health management
  • Align programs with system targets

Program Oversight
  • Ensures compliance with established clinical protocols and connectivity to the NM System and care continuum.
  • Leads initiatives including project proposal/ charter definition, budget development, resource allocation, management, information distribution and meeting facilitation and responsible for meeting operational, financial and strategic goals for unit, area, program and/or projects.
  • Develop team infrastructure including policies, procedures, development plans and annual reviews
  • Engages, empowers and involves employees in departmental goals and work culture initiatives


Qualifications

Required:
  • Bachelor's degree
  • 5-10 years leadership experience
  • Minimum 5 years' experience managing teams
  • Experience in population health, value-based care or care coordination

Preferred:
  • Master's degree in healthcare-related field
  • 5+ years clinical experience
  • Experience designing clinical programs
  • Knowledge of value-based care
  • Experience with analytics tools
