Director of Rehabilitation Services (PT or OT)
Western New York | Full-Time, Onsite LeadershipPay Range: $50.00–$55.00 per hour
An established skilled nursing and rehabilitation community in Western New York is seeking an experienced Director of Rehabilitation Services to lead its therapy department.
This is a working leadership position for a licensed Physical Therapist or Occupational Therapist who can balance clinical care with team leadership, regulatory compliance, department operations, and financial accountability. The Director will supervise rehabilitation staff, maintain an active clinical caseload, and partner closely with the facility Administrator and interdisciplinary leadership team.
The OpportunityThe Director of Rehabilitation Services will be responsible for the quality, compliance, productivity, and overall performance of the rehabilitation department. This leader will help ensure that residents receive coordinated, goal-driven therapy services that support function, independence, and quality of life.
The position maintains a clinical caseload with a minimum productivity expectation of approximately 35%.
CompensationThe anticipated pay range for this position is $50.00–$55.00 per hour, depending on experience, qualifications, and overall fit for the position.

Responsibilities What You’ll Lead as Director of Rehabilitation Services (PT or OT)Direct the daily operation of the rehabilitation department and supervise therapists, therapy assistants, rehabilitation aides, and assigned students
Participate in hiring, orientation, training, coaching, and development of rehabilitation employees
Oversee therapy evaluations, treatment plans, progress notes, care plans, discharge summaries, and other required documentation
Ensure therapy services comply with applicable federal and state requirements, professional practice standards, and facility policies
Collaborate with nursing, medical providers, social services, and other disciplines to integrate rehabilitation goals into each resident’s comprehensive care plan
Monitor clinical outcomes, department productivity, customer service, and financial performance; develop action plans when improvement is needed
Maintain oversight of therapy billing, reimbursement documentation, MDS-related responsibilities, and communication with the business office
Prepare and manage the department budget, including staffing, equipment, supplies, and service needs
Participate in care-planning, quality-assurance, safety, leadership, and other interdisciplinary meetings
Develop and provide education to employees involved in resident care
Maintain an active clinical caseload and provide evaluations and treatment within the scope of professional licensure
Current New York State license as a Physical Therapist or Occupational Therapist
Professional education through an accredited physical therapy or occupational therapy program, with a bachelor’s degree or higher
At least two years of supervisory or management experience
Current CPR/BLS certification
Demonstrated ability to organize departmental operations, delegate effectively, develop employees, and promote teamwork
Strong communication, decision-making, problem-solving, and organizational skills
Genuine interest in working with older adults and a commitment to protecting resident dignity and rights
Rehabilitation leadership experience within skilled nursing, long-term care, or a geriatric setting
Experience with therapy documentation, reimbursement, productivity management, budgeting, and interdisciplinary care planning
Proficiency with Microsoft Office and electronic clinical or therapy-management systems
