Director of Quality & Compliance

Elevate Home Health | Focus Home Health | Choice Home Health | San Diego Home Health

What You'll Do
  • Provide enterprise-wide leadership and oversight of quality, compliance, and performance improvement across all Elevate Home Health locations.
  • Lead and continuously strengthen the organization's Quality Assessment and Performance Improvement (QAPI) program.
  • Ensure ongoing compliance with CMS Conditions of Participation, Medicare requirements, OASIS requirements, applicable state and federal regulations, accreditation standards, and organizational policies.
  • Establish standardized quality and compliance practices across Focus Home Health, Choice Home Health, and San Diego Home Health locations.
  • Maintain organizational readiness for regulatory, accreditation, payer, and other external surveys and audits.
  • Partner with branch leadership before, during, and following surveys, including development and oversight of plans of correction when necessary.
  • Develop and oversee an enterprise-wide auditing and monitoring program focused on clinical documentation, regulatory compliance, patient care, and identified areas of risk.
  • Monitor and analyze clinical quality measures, OASIS-related performance, patient outcomes, hospitalizations, patient experience, and other key quality indicators.
  • Identify trends and areas of risk and partner with operational and clinical leaders to develop meaningful and sustainable improvement plans.
  • Lead or facilitate root cause analyses, corrective action plans, and performance improvement initiatives related to quality, compliance, patient safety, and adverse events.
  • Monitor complaints, grievances, incidents, adverse events, and other quality concerns to identify trends and opportunities for systemic improvement.
  • Support the organization's compliance program, including compliance monitoring, internal reviews, investigations, risk identification, and corrective action.
  • Lead or participate in the development and ongoing review of clinical, quality, and compliance policies and procedures.
  • Monitor regulatory changes and ensure new requirements are effectively communicated and incorporated into organizational practices.
  • Provide education, coaching, and regulatory guidance to clinical and operational leaders throughout the organization.
  • Develop meaningful quality and compliance reporting for executive leadership, identifying trends, risks, progress, and opportunities for improvement.
  • Partner cross-functionally with Operations, Clinical Leadership, Human Resources, Revenue Cycle, and other departments to strengthen organizational performance and compliance.
  • Travel to company locations as needed to support surveys, audits, education, performance improvement initiatives, and branch leadership.

You are comfortable moving between the details of a clinical record and the bigger picture of organizational performance. You can identify risk, communicate expectations clearly, build relationships with operational leaders, and turn findings into sustainable improvements.

Qualifications
  • Bachelor's degree in Nursing, Physical Therapy, Occupational Therapy, Healthcare Administration, Healthcare Quality, or a related healthcare discipline required; advanced degree preferred.
  • Current professional healthcare license preferred, with RN, PT, or OT licensure strongly preferred.
  • Minimum of 5 years of progressive home health experience, including experience in quality, compliance, clinical leadership, regulatory affairs, or performance improvement.
  • Strong knowledge of Medicare-certified home health operations, CMS Conditions of Participation, OASIS, QAPI, and Home Health Quality Reporting requirements.
  • Experience with regulatory and accreditation surveys, audits, plans of correction, and ongoing survey readiness.
  • Demonstrated ability to analyze quality and performance data and translate findings into actionable improvement strategies.
  • Experience developing and implementing corrective action and performance improvement plans.
  • Multi-site, regional, or enterprise-level quality/compliance experience strongly preferred.
  • Strong leadership, communication, analytical, and organizational skills.
  • Ability to effectively partner with clinical, operational, and executive leadership across multiple locations.
  • Quality, compliance, or home health certifications such as CPHQ, CHC, COS-C, HCS-O, or similar credentials are a plus.
  • Ability to travel to company locations as needed.
