Director of Home Health Services to join our team and oversee home health services across
all three Redstone Senior Living communities in Greensburg, Murrysville, and North Huntingdon.
This is a great opportunity for an experienced therapist who enjoys both
patient care and leadership and wants to play a meaningful role in helping older adults receive the right services at the right time.
In this position, you'll provide leadership and coordination for home health services throughout the three Redstone campuses, working closely with Directors of Rehabilitation, nursing teams, community leadership, physicians, residents, families, and home health providers to make transitions and care coordination as seamless as possible. You'll also remain connected to clinical care, providing direct patient treatment as workflow allows.

What You'll Do- Oversee and coordinate home health services across Redstone's Greensburg, Murrysville, and North Huntingdon communities
- Coordinate home health schedules across all three campuses to support timely and consistent service delivery
- Monitor referrals and help ensure evaluations, admissions, and follow-up visits are scheduled promptly
- Work closely with Directors of Rehabilitation, nursing teams, community leadership, and home health providers to coordinate resident care
- Help identify residents who may benefit from home health services
- Support smooth transitions between outpatient therapy, home health, wellness, restorative, and other therapy services
- Serve as a knowledgeable and responsive resource for residents and families navigating home health services
- Provide education to therapy teams, nursing staff, and community leadership regarding home health services, eligibility, and referral processes
- Complete and support OASIS documentation
- Provide direct patient care in the home setting, with a minimum productivity expectation of approximately 30% as workflow allows
- Monitor referral activity, scheduling, service utilization, and program performance across the three communities
- Identify opportunities to strengthen workflows, communication, and coordination between locations and service lines
- Support new home health programs and initiatives as they are developed
What We're Looking For- Graduate of an accredited therapy program
- Minimum of 3 years of experience in healthcare, rehabilitation, home health, scheduling, care coordination, or senior living
- Experience utilizing and completing OASIS
- Strong understanding of home health services and the continuum of care
- Experience coordinating schedules, referrals, and interdisciplinary services is preferred
- Strong leadership, organizational, communication, and customer service skills
- Someone who can manage multiple priorities while staying responsive and organized
- Comfortable working independently while also collaborating closely with teams across multiple locations
- Proficiency with electronic medical records, scheduling systems, and Microsoft Office
- Ability and willingness to travel between the three Redstone campuses and other service locations as needed
