Full Job Description
Director of Clinical Services of Home Health

The Director of Clinical Services is responsible for the overall direction of home health clinical services. This position implements, evaluates goals and objectives for home health services that meet and promote the standards of quality and contribute to the total organization and philosophy.

Essential Possible Job Functions:
  • Coordinates and oversees all direct and indirect patient services provided by clinical organization personnel.
  • Provides guidance and counseling to coordinators and Clinical Supervisors to assist them in continually improving all aspects of home health care services, provided through organization personnel.
  • Assists Clinical Supervisors in managing clinical teams and planning.
    Provides help in assessment, planning, implementation and evaluation of patient and family/caregiver care to all clinical personnel as indicated.
  • Evaluates performance of Clinical Supervisors.
  • Assists Clinical Supervisors to develop skills and techniques in evaluating the performance of clinicians.
  • Hires, evaluates, and terminates organization personnel.
  • Conducts clinical performance evaluations annually, or more frequently if indicated.
  • Oversees the maintenance of patient clinical records, statistics, reports and records for purposes of evaluation and reporting of organization activities.
  • Assures proper maintenance of clinical records in compliance with local, state and federal laws.
  • Responsible for the maintenance of adequate and appropriate inventory supplies and equipment for the provision of patient services.
  • Assures for the quality and safe delivery of home health services provided through the Organization.
  • Assures compliance with all local, state and federal laws regarding licensure and certification of organization personnel and maintains compliance to the ACHC Home Care standards.
  • Stays informed about changes in the field of nursing and home health care, shares information with appropriate organization personnel.
  • Leads case conference and the clinical staff directly.
  • Promotes home health referrals in the health care community.
  • If functioning as Clinical Manager: Will be available at all times during operating hours and will provide oversight of all patient care services and personnel including making patient and personnel assignments; coordinating patient care; coordinating referrals; assuring patient needs are continually assessed: and assuring the development, implementation, and updates of the individualized plan of care.
  • Other duties as assigned by the Senior Director/Administrator.
  • Demonstrates C.A.R.E. values to our clients, family members, customers and staff.
  • Complies with the company's privacy practices/procedures and all state/federal privacy laws as outlined by HIPAA related to client and employee records, to include but not limited to information accessed through any company web-based system and/or electronic medical records.
  • Demonstrates teamwork and prompt and regular attendance to work to ensure that quality care and services are provided to the clients we serve.


Supervisory Responsibilities
  • Demonstrated ability to supervise and direct professional administrative personnel.

Special Requirements
  • If making home visits, must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.

Education & Qualifications

To perform this position successfully, an individual must be able to perform each essential duty satisfactorily. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
  • A degree in Medicine or Bachelor's degree in Nursing from an accredited program by the National League for Nursing. Master's degree in health care preferred.
  • Three years of management experience in a home health or related health care organization.
  • Knowledge of business management, governmental regulations and ACHC standards.
  • Has been oriented to an Administrator job responsibilities.
