Director of Care Transition Qualifications:
• Must have at least 2 years of experience in a healthcare setting.
• Be knowledgeable with Microsoft Office (Outlook, Word, Excel).
• Must be able to maintain strong working relationships with hospital Social Workers, Doctors and other members of the healthcare professional community.
• Must have patience, tact, cheerful disposition, and enthusiasm, as well as must be willing to handle residents, staff, and visitors, based on whatever maturity level at which they are currently functioning.

Director of Care Transition Job Overview:

The Hospital Clinical Liaison is responsible for developing and cultivating new patient prospects for admission to the facility. The incumbent will be responsible for maintaining relations with community partners, family members, and facility personnel.

Director of Care Transition Responsibilities:
• Interview residents/guardians/sponsors and obtaining required admission information.
• Assist in resident admission orientation program.
• Maintain an accurate record of available beds and census.
• Abstract information from records as authorized/required for insurance companies.
• Notify staff when the resident has arrived at the facility to escort the resident to assigned room.
• Maintain confidentiality of resident care information including protected health information.
• Report known or suspected incidents of unauthorized disclosure of such information.
• Develop, schedule, and guide tours/personnel throughout the facility.
• Keep a supply of brochures and other printed materials readily available.
• Maintain an adequate liaison with families, residents, residents, and the community.
• Plan and implement special events that serve to advance staff, resident, and community relations.
• Work closely with admissions to insure a smooth care transition
